Resources

Materials

  • MyMedList - A pdf tool for tracking your medications.
  • iPhone App for tracking you medications - Create an electronic record of the medications for you and your family, along with the prescriber and pharmacy information associated with those medications. You can then print or email this to share with doctors, pharmacists and other healthcare providers. This is a good tool to help prevent adverse reactions and interactions between the medicines, foods or supplements. To maintain privacy, the information is fully controlled by the user and is only recorded on the user’s electronic device. This app is based on the MyMedsList created by the Tucson-based Critical Path Institute.
